Please inform the practice if you change your name, address or telephone number. This helps to prevent embarrassing situations such as sending letters to the wrong address. We pass on address and basic information to hospitals (through referrals) and the Health Authority so that they can also fulfill their obligations.

It is surprising the number of people who have waited a long time for a hospital appointment only to find that the appointment was sent to the wrong address. We are hoping to implement a text reminder service for appointments so please ensure that we have an up to date mobile number on your records. This has been shown in other practices to significantly reduce non attendance rates.
